Edberts House is a thriving community charity currently managing four community projects based in East Gateshead. We work together with local residents of all ages to make our communities happier, healthier and more welcoming places to live.

Over the past five years our Community Linking Project has contributed to the health and wellbeing of patients in Gateshead by linking them to a range of community services. We now have an exciting opportunity for Community Link Workers to join our team. Working across a Gateshead Primary Care Network (PCN) you will support patients who have visited the GP by providing advocacy, signposting and accompanying individuals to local, non-medical sources of help. You will proactively build relationships with patients and community organisations to support individuals to come up with innovative solutions to problems.

Candidates will come from a community, information/advice, social care or health background, must be excellent communicators and able to work in a multi-disciplinary team.

 

An enhanced DBS is required for this post.
