We have a new and exciting opportunity for the right person to help make a difference to patients’ well-being across the Wansbeck Primary Care Network. This is a ‘social prescribing’ role and is part of the new innovative and evolving dynamic approach in delivering health and social care to patients in the region.
We are seeking a highly motivated, passionate, and suitably experienced person to lead our new team of Community Link Workers (Social Prescribing). Our Team Leader will provide line management support and supervision to the link worker team, provide advice and support on the role, as well as manage a caseload of patients with a range of support needs. They will also liaise with the various partners and steering group on behalf of the Community Link Workers they manage.
Our Community Link Workers will give time to support patients to focus on ‘what matters to me’ and take a holistic approach, connecting people to community groups and statutory services for practical, emotional, and social support.
The successful applicant will have excellent communication skills, be able to work independently and develop effective relationships with service providers, patients, and colleagues, as well as being comfortable in leading group discussions.
