Hospital Discharge Worker

Fixed Term Contract until 31st March 2020

(With the possibility of an extension)

21 hours per week, £16,562 pro rata per annum

This post will provide short-term practical help to older people returning home after discharge from hospital. A flexible approach to work is required. You must demonstrate empathy for older people and an understanding of issues that affect them. Possess a full, clean driving licence and have access to a car for work is essential.

Post Hospital Discharge Worker

Responsible To: Client Services Manager

Location: Sunderland Royal Hospital

Purpose of Post

To work with the Client Services Manager to implement a successful hospital to home discharge service to promote enablement for people aged 60 and over without a formal care package or carer.

Job Description

  •  To visit older people in their homes to encourage practical support after their discharge from hospital
  •  Report to the Client Services Manager with issues concerning Service Users on and after discharge
  •  To promote and encourage Service User participation wherever possible
  •  To liaise with Discharge Nurses and Social Work teams to target appropriate referrals.
  •  Signpost Service Users to appropriate services and agencies
  •  Keep Service Users records up to date and accurate
  •  To advertise and promote the project
  •  Assist in an effective monitoring and evaluation system
  •  Assist in monitoring health outcomes of service users
  •  To maintain strict confidentiality at all times
  •  To abide by and implement all policies and procedures of Age UK Sunderland, including being aware corporately and as an individual for Health and Safety.
  •  To undertake all reasonable tasks in keeping with the level of responsibility of the post as requested by the Director

Essential Criteria

  •  Have experience of working with older people
  •  Have experience of working with volunteers
  •  Be able to work flexibly and with motivation
  •  Have excellent verbal and written communication skills.
  •  Have experience of providing a service to older people
  •  Have excellent organisational skills
  •  Have record keeping skills
  •  Be honest, reliable, and trustworthy and maintain confidentiality at all times
  •  Be I.T. literate with experience of word processing and database
  •  Possess a full, clean driving licence and have access to a car for work is essential.
