Hours: 30 hours per week

BLISS=Ability is a user-led charity based in South Tyneside that offer services and support to vulnerable, isolated, disabled people, and people living with a long term health condition.  Adopting a fully inclusive approach we are committed to improving people’s health and wellbeing by providing person-centred support, information, advocacy and training to enable people to become independent and enhance their quality of life.

With funding from the National Lottery Community Fund, and working in partnership with ACTS (Age Concern Tyneside South) and YVC (Your Voice Counts), we aim to work with local health and social care, and voluntary sector organisations to develop our Social Prescribing Project to reach and support people who have a mixture of needs at different levels. 

 

1.The Role

We have an exciting opportunity for an experienced and self-motivated individual to work as a Link Worker for our Social Prescribing Project.  The Link Worker who will be integral to the Social Prescribing project, and will help people with a wide range of health and social care needs They will support people to create their own action plan, focused on non-clinical aspects of mental and physical wellbeing, and support them to engage with appropriate local community activity and opportunities to meet their needs, and interests, to improve their health and wellbeing. 

The ideal candidate will have an interest in working with people with complex social and health needs, be committed to supporting individual decision making and a person-centred approach.

2.Responsibilities
  • Liaise with health and social care services, including local authority, primary care, welfare services, and VCSE organisations.
  • Maintain an active caseload of clients, and keep accurate records.
  • Provide face to face consultations / meetings with clients.
  • Develop a person-centred action plan with clients.
  • Assist clients to identify services and activities that will reduce social isolation and inactivity, therefore promoting mental and physical wellbeing.
  • Keep accurate and up to date records.
  • Arrange, and conduct, regular reviews with clients.
  • Assist in the monitoring and evaluation of the service, including the collection of monitoring information.
  • Attend and contribute to feedback and evaluation sessions with the aim of improving the service.
  • Assist in the promotion of the service.
  • Input information onto spreadsheets / databases.
  • Ensure the confidentiality of client’s records.
  • Attend all relevant training sessions, and external meetings.
  • Attend staff meetings and in-house training sessions.
  • When necessary to work outside normal office hours and to travel within Tyne & Wear.
  • Other duties and shared tasks as and when required.

4.Terms and Conditions

Salary:                       £20500 p.a.  pro rata

Contract Period:          Funding for this post is to 1 May 2021

Place of Work:            BLISS=Ability, 34-36 New Green Street, South Shields   

Hours of work:            30 hours per week

Holiday entitlement:    Refer to Terms and Conditions of Employment for details.

Notice Period: Probationary period of 3 months before offered a permanent contract during which a months’ notice is applicable.  After that a written notice of not less than 1 month.

Travel Expenses:         Paid at BLISS=Ability current standard rate of 45p per mile.
