Location: Owing to the current pandemic, these roles are required to work flexibly between home, GP practices and an office location
We are looking or experienced, passionate people who can contribute to the delivery of a person-centred, flexible and dynamic Social Prescribing service in two of the Primary Care Networks in Middlesbruogh.
