Social Prescribing Link Workers

Location: Within GP Practices across Middlesbrough, Stockton and Redcar

We’re looking for a new team of special people to help us develop and deliver social prescribing in local GP Practices. Social prescribing is a way of helping people to address their health and wellbeing needs in a holistic way and it aims to support individuals to take greater control of their lives.

You will be responsible for ensuring the service supports patients to take control of their wellbeing and live happier, healthier lives by raising awareness and understanding, co-producing personal support plans, working closely with GP practice staff and developing strong connections in the community.
