Social Prescribing Locality Lead, North Tyneside

Are you an experienced practitioner and experience leader with a good understanding of personalised care, the wider determinants of health and how the health and care system currently works? 

We are looking for a compassionate leader to lead a team of Link WorkersLink Worker Coaches, Psychosocial Link Workers and Care Coordinators working across North Tyneside. As a compassionate leader you will pay attention to the challenges faced by the team and the people we work so that we can make sense of and understand them together. You will engage in coaching conversations that support staff to discover and test solutions themselves. You will empathise with those you lead, providing a safe environment in which staff can take safe risks, test out new ideas and fail – which is a necessary step in learning and innovation. You will take thoughtful and intelligent action that engages and involves staff along the improvement process of ideation, evaluation and implementation. 

You will have experience of monitoring service performance against contractual targets and ensuring that the service is visible and accessible to referring partners, self-referring people and the local community. 

As a fantastic collaborator, you’ll be great at developing partnerships with your peers, their teams, external stakeholders and commissioners, working together to support the communities we work with.
