Owing to the current pandemic, this role is required to work flexibly between home and an office location.

We’re looking for an experienced manager to lead a team of Social Prescribing Link Workers who are delivering a person-centred, flexible and dynamic Social Prescribing service in five Primary Care Networks across Teesside.

You will be responsible for ensuring the service is safe and effective but also helping to develop the service in partnership with GPs and Practice Staff.
