We have a brand new role for a Social Prescribing Worker. You will use your skills in person centred support, project development and partnership working to make a real difference.

You will play an instrumental role in the development of the project within the GP practice, supporting patients to improve their wellbeing and connect with other services and community activity.

Ageing Better Middlesbrough is a Big Lottery funded programme aimed at reducing loneliness and isolation in Middlesbrough for people aged fifty and over.

Middlesbrough and Stockton Mind is the lead organisation for this innovative and exciting programme and we are now working in partnership with Linthorpe Surgery to develop Social Prescribing in Middlesbrough.
