
Working as part of Darlington Mind's busy Supported Living Team, assisting people staying within our communal and independent properties with a range of practical and mental health support needs to lead a more healthy lifestyle and offer advice and/or information where required.
The role can make a real difference to the lives of vulnerable people, helping them to find their feet, develop skills and emotional resilience in a safe and supportive environment. Ideally we are seeking candidates with previous experience of work in social housing and/or supporting adults with mental health problems who can work without direct supervision to:
assist our tenants to develop life and independence skills, monitor their welfare and assist the Housing Support Coordinator in reviewing the individual's support plan & risk assessment
liaise with relevant care and/or other relevant key professionals in relation to the Tenants’ well-being
keep good quality/accurate records of daily contact with tenants to support the service and our duty of care for service-users including for adult safeguarding
assist the Co-ordinator at relevant meetings internally & externally
assist the Coordinator in assessing and processing new housing applications
assist tenants with identifying and applying for personal & housing benefits
assist with recording & accounting of Tenants Accommodation Fee’s/Rent
assist tenants in finding out about and accessing other appropriate services within Darlington Mind or elsewhere which support their needs
data entry onto the company database and assisting with the production of statistical/outcome data, reconciliation of accounts and any other relevant information required internally and externally
attend and complete appropriate training courses
any other duties that are compatible with the aims and objectives of this post.
