We are a local Charity working with people who are blind, partially sighted and or D/deaf, hard of hearing on their journey to independence. We ensure people have choice and control over their lives and help others better understand the needs of people with sensory loss.

We deliver early intervention, crisis prevention and integrated social activities to reduce preventable sensory loss, maintain independence and mental wellbeing and reduce social isolation. Our interventions ensure that people;
- can live more independently,
- are less socially isolated,
- are more confident and empowered,
- experience improved wellbeing.

Our unique integrated Pathway to Independence makes sure we provide the right support at the right time for individuals. Our interventions include;
- Personal Assessments and Planning
- Peer Support
- Vision Rehabilitation
- Digital Enablement
- Skills and employability

Our Team, comprising specialists within the field and those with lived experience, provides tailored support for individuals and works with a wide range of external organisations to maximise people's experience. Vision25

Vision25 is based in Stockton-on-Tees, one the most 10% deprived areas in England, the group is a great place to be, we are young adults and our average age is about 23 years old. We all have learning disabilities and some of us have a profound and multiple learning disabilities e.g. Downs Syndrome, Cerebral Palsy, Autism and other Chromosome Abnormalities. When we finished college at 19, there was no-where to go, we were quite isolated at home. One of our teachers Jen Addison had a brilliant idea to open a centre where we could get together, make friends, learn independent living skills and be part of the community. We started as a Saturday club and 2 years later we are open 6 days a week.
One of our aims at Vision25 is to support the group in work related learning skills. We do this by focusing on the following areas:
