Job Title: Self Directed Support Lead
Reports to: Project Team Colleagues and CEO

•Increase the control people have over their lives and support (personalsation)
•Enable people we support to play their part in the world and be seen as valued, contributing citizens
•Encourage staff to be creative, flexible and person centred
•To support people to control their own support
•To support people and their teams to come ‘un-stuck’
Location: Whitley Bay and working alongside people in their own homes in North Tyneside, Newcastle and Northumberland.
Working Hours: 37hrs per week, usually Monday to Friday but flexible to the needs of the organisation.
Responsibilities and Duties of the Job Role-specific Duties:
· To support people and their staff to plan for new opportunities, sticking with people until the changes are embedded.
· To support people who employ their own Personal Assistants via our Shared Management offer.
· To research and build connections with community projects and organisations so that people we support can get involved.
· Ensure accurate records are kept
· Be responsible for promoting and safeguarding the welfare of people supported by New Prospects
Working with Others:
· Build good relationships with families /friends of people we support.
· Be a great team player, you’ll be part of a small, self-managing team.
· Communicate well with social workers and other professionals involved in people’s lives and support.

· Seek opportunities for personal and professional growth
· Be a role model for other Support Workers and be an ambassador for the new Prospects.

Personal and Professional Development:
