First Stop Darlington (FSD) and Tees Valley YMCA (TVYMCA) each have an exciting new post for a joint project funded by the Primary Care Network and in partnership with Darlington Healthwatch Darlington (HWD).
We are each looking for enthusiastic and caring individuals with a passion for supporting people struggling with their mental health. We need people who are comfortable and confident to work on their own but are also dynamite at team work.
This project is new and funded through the Primary Care Network. Both posts will work closely with GP Surgeries and offer support to patients identified by healthcare professionals as needing additional support to engage effectively with their treatment plan, or to support them while they await appointments with mental health services. These roles will also include work in supporting people to attend their first appointments to help them be comfortable in a new surrounding and hopefully increase their engagement and successful outcome.
The TVYMCA will engage and support young people aged 16-25 years to access meaningful activities, to develop resilience skills and improve coping mechanisms. The worker linked to TVYMCA will, where possible adopt a whole family holistic approach to improving the wellbeing of the young person. TVYMCA have many projects to help the individual to improve their confidence and mental health.
The worker linked to FSD will liaise with our Mental Health Support Worker to identify and recruit individuals that will benefit from our Mindset Programme and Support Groups. This programme is designed to support people with past trauma to work towards leaving that trauma in the past and moving forward with a positive mindset about their future. They will also adopt a whole family approach where appropriate, working alongside the TVYMCA worker.
Both posts will be responsible for putting together a plan to help individuals engage with the best services to meet their individual needs. Individuals will be expected to do some weekend and evening work, though this will be kept to a minimum.
