About the Role
Durham County Carers Support is seeking a compassionate and motivated Carer Support Co-ordinator to join our dedicated team. You will play a vital role in supporting unpaid Carers across County Durham — offering information, advice, and guidance to help them make informed choices and improve their health and wellbeing.
This role will have a particular focus on supporting Carers who have, or who care for someone with, mental health needs in East Durham. You’ll help ensure these Carers receive the understanding, specialist support, and practical help they need.

Key Responsibilities

Provide information, advice, and emotional support to Carers via telephone, email, video call, and in-person meetings.

Offer one-to-one support tailored to each Carer’s situation, with an emphasis on Carers affected by mental health needs.

Help Carers apply for NHS Carer Break funding and Carer Emergency Support.

Carry out Carer registration and initial assessments using the Carers Outcomes Tool.

Manage a personal caseload and maintain accurate, up-to-date records on Charitylog.

Refer Carers for full statutory assessments where appropriate.

Work closely with GP surgeries, mental health teams, social care services, and community organisations to identify and support hidden Carers.

Offer practical help with welfare benefits and grant applications.

Keep informed about relevant legislation, welfare benefits, and local support services.

Work collaboratively with colleagues to share skills, knowledge, and best practice.

Occasionally work evenings or weekends (TOIL provided).
About You
You’ll have a caring and empathetic approach, with the ability to work independently and as part of a supportive team.
Essential:

Understanding of the challenges faced by Carers, particularly those affected by mental health needs.

Knowledge of legislation and policies relating to Carers.

Experience supporting people to access welfare benefits or grants.

Knowledge of local statutory and voluntary services.

Experience working with vulnerable adults.

Excellent communication, IT, and record-keeping skills.

A-level (or equivalent) qualification or above.

Full driving licence and access to a car.
Desirable:

Qualification in Health or Social Care.

Experience working directly with Carers.

Counselling or listening skills.
