A multi-professional assessment team supported by the Better Care Fund is helping vulnerable people in Stockton to stay in their own homes. The seven-strong wellbeing team, which includes seconded nurses, physiotherapists and social workers, is piloting a holistic assessment addressing people’s social care and welfare needs.
The service is jointly commissioned by the CCG and local authority and delivered by the local authority and foundation trust.
