Last year, Northumberland was awarded Vanguard status by the NHS. Health and care partners are working across Northumberland to support the 'joining up' of services by creating a fully integrated 'primary and acute care system'. This will build on successful work which has taken place over many years to integrate health and social care across the county. The Vanguard aims to empower local communities to live longer and healthier lives at home, increase focus on prevention and ensure a seamless, high quality experience when people need to access health and care services.
This month, the Vanguard are beginning the first phase of engagement, seeking people’s views about how they currently access services and what they would like to see improved.
